MEMORANDUM FOR MADE A MISTAKE, AMN, FR123-45-6789

FROM: 4 XXX/CC

SUBJECT: Letter of Reprimand - Disobey an Order

1.  Saturday, April 6, 2003 you failed to obey a lawful order given to you by Capt Dilbert. He
specifically informed you to cleanup the ###th Communications building.

2.  Air Force Instruction (AFI) 36-2618, The Enlisted Force Standards states:

“As member of the professional of arms, all enlisted members are sworn to support and
defined the Constitution of the United States and to obey the orders of all officers
            appointed over them.”

3.  You are hereby reprimanded. You have engaged in aberrant behavior which will not be
tolerated from a member of the United States Air Force. Your past behavior has raised serious
questions about your ability to remain in the United States Air Force. I want to know that I
consider disobeying a lawful order to be one of the most serious offenses an Air Force member
can commit. Strict adherence to orders is the only way that any military force anywhere in the
world can ever execute its mission. Your unit is not different; you have damaged its mission
capability by identifying yourself as one who cannot be depended upon to perform as ordered.
You are going to have to strive diligently if you ever hope to reestablish your dependability. A
repetition of this totally unacceptable behavior will result in stronger action against you.

4.  PRIVACY ACT. AUTHORITY: 10 U.S.C. 8013. PURPOSE: To obtain any comments
or documents you desire to submit (on a voluntary basis) for consideration concerning this
action. ROUTINE USES: Provides you an opportunity to submit comments or documents for
consideration. If provided, the comments and documents you submit become a part of the action.
DISCLOSURE: Your written acknowledgment of receipt and signature is mandatory. Any
other comment or document you provide is voluntary.

5.  You will acknowledge receipt and return this letter to me within 3 workdays of your receipt.
Your signature on this document is solely for receipt purposes and is not an admission of guilt.
Any comments or documents you wish to be considered concerning this LOR must be included
with your response.
